Repeat Bulk food for customer self-service not properly labeled: observed bakery items displayed for customer self-service that had ingredient information for various components of the items but not the finished product, e.g. ingredient information provided for donut, filling, and icing separately, but not for the chocolate iced custard filled donut in the case. Please provide identity labeling in the case, and ingredient information for finished products.

Observed no documentation of food employees having been notified of their responsibility to report information about their health. Please inform food employeesin a verifiable manner of their reponsibility to report information about their health as it relates to illnesses transmissible through food. Provided employee health information to the person in charge during inspection.
3717-1-04.6(B)/Sanitizing frequency of utensils and food-contact surfaces - before use and after cleaning
Food-contact surfaces of equipment not sanitized at the required frequency: probe thermometer not sanitized between uses. Please begin cleaning and sanitizing thermometer probe between uses.
3717-1-04.5(A)(1)/Cleanliness of equipment food-contact surfaces and utensils.
Equipment food-contact surfaces or utensils are unclean: undersides of creamer dispenser and capuccino machine, and scissors used to cut food packages open. Please begin cleaning food-contact surfaces at least every 4 hours while in use.

Refrigerated, ready-to-eat, time/temperature controlled for safety (TCS) foods not properly date marked: Polish sausage in the walk-in cooler was date marked for use or discard within 8 days. Please ensure TCS foods requiring date marking are properly marked for use or discard within 7 days; the date the original package is opened is day 1.

Unnecessary chemicals stored on premises: observed weed killer in the utility room. Please remove weed killer from the establishment.
3717-1-04.1(Y)/Temperature measuring devices.
Thermometer was missing from cooler(s) used for storage of time/temperature controlled for safety (TCS) food and beverages. Please permanently place a thermometer in each cooler used for TCS food and beverage storage, and locate them in the warmest part of the unit and where they can be easily viewed.

Outer opening not protected: observed a gap under the south door. Please take steps necessary to adequately protect the outer opening from entry by pests.
3717-1-04.8(E)(3)/Single-service articles and single-use articles - storage.
Single-service and single-use articles not protected from contamination: observed unwrapped coffee filters stored unprotected on top of brewer, and coffee spills on single-service cups and lids in multiple cabinets under the beverage self-service station. Please discard contaminated/soiled items and begin storing single-use articles in a clean, dry location not exposed to splash, dust, or other contamination, and in the original protective package or by using other means that afford protection from contamination until used.

Dispensing tube on bulk half and half container not cut properly. Please begin cutting dispensing tube on the diagonal and leaving no more than 1" protruding from the chilled dispensing head.
3717-1-04.1(A)/Equipment and utensils - durability and strength.
Non-durable equipment observed: some wire shelving in walk-in coolers has begun to rust. Please remove rust and restore smooth, durable, easily cleanable, and non-absorbent surface finishes.
3717-1-04.1(H)/Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces not easily cleanable: observed basket that is not cleanable used to store donut bags, accumulation of frost inside the ice cream freezer, and cardboard liners that are not cleanable used on some shelving. Please replace basket with a container that is cleanable, defrost ice cream freezer, and remove cardboard shelf liners.

Equipment not properly elevated or sealed to floor: mop sink. Please seal mop sink to floor.

Facility not maintained clean: observed unclean floors in the utility room; and mold/mildew on floors and walls and heavy accumulation of dust/dirt on condenser fan guards in the south walk-in cooler. Please clean affected surfaces.
