Critical Corrected During Inspection Did not observe any form of an employee illness agreement anywhere in the facility. License holders must require employee(s) to report symptoms, diagnosed illness by a health care provider, or high risk situations as specified in rule. Have each employee sign a copy of the illness agreement provided during inspection and keep on file.

3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Observed dirty can opener in back of kitchen. Food must be properly protected from contamination by cleaning equipment and utensils thoroughly. Clean can opener frequently to prevent cross contamination of food.

3717-1-02.4(C)(17) / PIC: duties - ensure the facility has written procedures for responding to vomiting and diarrheal events.
Corrected During Inspection No written procedures for responding to vomiting or diarrheal events observed in the facility. The food service operation or RFE shall have written procedures for employees to follow when responding to vomiting or diarrheal events that involve discharge onto surfaces in the food surfaces in the FSO or RFE. Keep the written procedure provided during the inspection in the kit constructed by PIC.

3717-1-06.2(I)(1) / Lighting - intensity (10 FC)
Observed burnt out light in walk-in cooler. The light intensity shall be at least ten foot candles at a distance of thirty inches above the floor, in walk-in refrigeration units and dry food storage. Replace light bulb so that light intensity is at least ten foot candles in walk-in cooler.
