[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(A)(2) / PIC - Level Two Certified Manager
Observed that the facility could not find its certificate for a state approved level 2 food safety training. At least one person in a supervisory role shall have state approved level 2 food safety training.

3717-1-04.2(G)(1) / Food temperature measuring devices - availability
Observed that the facility could not locate a metal stem thermometer for foods. Purchase metal stem thermometer to accurately check food temperatures.

3717-1-04.2(I) / Sanitizing solutions - testing devices.
Repeat Observed that the facility could not locate its sanitizer test strips. The sanitizer was properly concentrated at the time of inspeciton. Purchase QT-40 test strips to accurately measure the sanitizer concentration.
