[?] A summary of the violations found during the inspection are listed below.

3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Observed raw eggs and raw bacon being stored above cooked food in cooler in middle of store. Food must be properly protected from contamination by separation, packaging, and segregation from raw animal food. Place raw eggs and bacon on the bottom of the cooler to protect cooked foods.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Observed gaskets on cooler doors 1 and 2 that were damaged. Equipment components must remain intact, tight or properly adjusted. Have gaskets repaired so that coolers can properly maintain temperature.

3717-1-05.4(B) / Outdoor storage surface.
Outdoor refuse storage surface not properly constructed and sloped to drain. An outdoor surface for refuse shall be constructed of nonabsorbent material such as concrete or asphalt and shall be smooth, durable, and sloped to drain. Place dumpster on smooth surface such as concrete.

3717-1-05.4(N) / Covering receptacles.
Observed outdoor waste receptacle that was not closed with a tight-fitting lid. Receptacles for refuse, recyclables, or returnables shall be kept covered with tight-fitting lids or doors if kept outside the food service operation or retail food establishment. Close lid of dumpster properly after every use.

3701-21-25(I) / Person in charge certification in food protection
RFE did not have a person in charge with the PIC certification in food protection. FSOs and RFEs must have a person in charge with an Ohio Department of Health issued food safety certification.
