[?] A summary of the violations found during the inspection are listed below.

3717-1-04.4(A)(3) / Equipment - cutting or piercing parts of can opener not maintained.
Observed can opener blade is dull and has a build up of debris. Clean the can opener often and replace the blade to prevent metal shavings from building up.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Observed that the facility had a fan in the rear (not in use) that was covered in dust. Clean nonfood contact surfaces thoroughly and often to prevent any dust build up which may contaminate in kitchens.

3717-1-06.4(A) / Repairing.
Observed that the facility had cove molding missing near the rear door. Observed a hole around the floor drain. Observed holes in the walls behind the mop sink. Observed browned or damaged ceiling tiles Maintain floors, walls, ceilings in good repair to prevent any sources of contamination or access to facility from pests..

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Observed debris build up along the walls and floors in the facility. Scrub walls thoroughly and often to prevent build up.

3717-1-06.2(I)(1) / Lighting - intensity (10 FC)
Observed that the light in the walk in cooler worked, but the light switch had to be propped at a very specific angle. Repair the cooler light switch so that adequate light is provided in the walk in cooler.

3717-1-06.1(I) / Light bulbs - protective shielding.
Repeat Observed some lights in the rear of the facility had cross grated shields that would allow broken shards of glass to fall through. Replace light shields in the food prep areas with full plastic to prevent any accidental contamination of foods in case of an accident. Recommend switching grates with the customer service area.

Went over the report with the PIC. At the time of inspection the facility had areas where cleaning needed to be done more often/ more thoroughly, but overall from the prior inspections the facility has made many steps forward. The shelves have been replaced, the pizza racks have been cleaned. Continue to clean thoroughly and often underneath shelving and equipment throughout the kitchen.
