[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(A)(2) / PIC - Level Two Certified Manager
Repeat Facility does not have a ODH Level II food safety certificate. ServSafe classes have been completed, and certificates on site, but no ODH certificate. Showed example to the manager, Robert, who said he will look for a copy and send it to me via e-mail if he found it. I gave him a list of our offered classes for 2019, for anyone interested and for him to renew his certification.

3717-1-04.1(Y) / Temperature measuring devices.
Repeat Observed no thermometer in the 2 door prep cooler to measure ambient air temperature.

3717-1-03.2(M) / Wiping cloths - use limitation.
Repeat Observed sanitizer wiping cloths to be stored on top of prep tables and equipment, rather than in a sanitizer solution.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Repeat Observed duct tape throughout the facility on equipment, countertops, handles, etc. Duct tape is not a smooth, durable, and easily cleanable surface.
