3717-1-02.2 (B) / Hands and arms: cleaning procedure
Critical Employee did not wash hands for 20 seconds before rinsing off their hands. All employees must wash hands for at least 20 seconds before rinsing.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Cutting board surface was heavily stained from use over several days. All food contact surfaces must be regularly washed, rinsed, and sanitized, maintained clean to sight and touch.

3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat No proof of an employee having Manager Certiciation in Food Protection was present at facility. At least one employee must have Manager Certification, and proof of certification must be available upon request at the facility.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Gasket on door to reach-in cooler was damaged and needs to be replaced or replaced. All door gaskets must be intact to ensure an appropriate seal between door and cooling unit.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat There was excessive frost build-up in reach-in freezers in the front and back which need to be defrosted or scraped clean. All non-food contact surfaces must be maintained clean to sight and touch.

3717-1-06.1(L) / Toilet rooms - enclosed.
Repeat Toilet room door was not self-closing. All toilet rooms that open onto food prep areas must be self closing.
