3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Observed pizzas in freezers made and sitting inside freezer exposed.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.
3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Corrected During Inspection Observed no date on meatballs that were made in prep cooler. Observed no date on bacon removed from original packaging in prep cooler. Observed no date on items removed from original container or coming from opened deli meat (deli meats used for pizzas put in seperate bag, green peppers, etc). Observed date marking in deli meat cooler not consistent- PIC states that they discussed having to be consistent in the future.
To prevent foodborne illness, refrigerated, ready-to-eat, TCS food held at a temperature of 41°F or less for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed, sold, or discarded. The day the food is prepared or opened is day one.
3717-1-03.5 (C)(2)(e) / Food labels - label information
Critical Observed sandwiches in cooler with name of item, where it came from, and ingredient list. However, sandwiches in cooler were missing the allergin label.
To prevent exposure to food allergens, food label information shall include the name of the food source for each major food allergen contained in the food unless the food source is already part of the common or usual name of the respective ingredient.

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Corrected During Inspection Observed sauce containers in prep cooler with no label. Observed various meats in prep cooler not easily identifiable with no label.
Working containers holding food or food ingredients that are removed from their original packages shall be identified with the common name of the food, except containers holding food that are readily and unmistakably recognized.
3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Corrected During Inspection Observed eggs and bacon sitting on floor in walk in cooler. Observed molded cheese and expired milk on floor in cooler- even items being sent back need to be stored 6 inches above the floor and need labeled with Do Not Use or something indicating that they are going back.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor.
3717-1-04.4(B) / Cutting surfaces.
Corrected During Inspection Observed cutting board with scores that can not be cleaned. PIC states that cutting board is not used and labeled the board as do not use.
Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.
3717-1-04.8(G) / Kitchenware and tableware - handling.
Repeat Observed straws single-use item sitting in a cup not individually wrapped or dispensed. Owner states that they will be switching to wrapped. This is to prevent bare hand contact.
Single-service articles or single-use articles and cleaned and sanitized utensils shall be handled, displayed, and dispensed so that contamination of food-contact-and lip-contact surfaces is prevented as described in this rule.
3717-1-05.4(H) / Toilet room receptacle - covered.
Observed trash can in bathroom that women use that is not covered.
A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
3717-1-06.1(M) / Outer openings - protected.
Observed exterior door leading to dumpsters with light that can be seen under the door.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.
3717-1-06.2(I)(1) / Lighting - intensity (10)
Observed reach in prep cooler on bottom with no lighting and food items in back of cooler that can not be seen. Lights will be needed in cooler to not only see food items but also cleanliness of cooler.
The light intensity shall be at least ten foot candles (one hundred eight lux) at a distance of thirty inches (seventy-five centimeters) above the floor, in walk-in refrigeration units and dry food storage areas and in other areas and rooms during periods of cleaning.
3717-1-06.4(A) / Repairing.
Observed door seal to walk in needs replaced. Observed scrapping marks on walk in cooler door. Observed wall with deer head has holes and scrapping. Observed side of counters peeling off or missing with tape covering. Observed cabinet behind counter that does not close.
The physical facilities shall be maintained in good repair.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed fans in walk in cooler with dirt/debri build up. Observed walk in cooler with debri/dirt build up under shelving and on floor. Observed splatters on wall and ceiling near walk in. Observed splatter marks on wall by water heater. Observed bottom of coolers/fridges/freezers with food debri and crumbs. Observed bottom of prep cooler stained.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Observed small oven not being used with broken handle. Observed ice cream cooler not being used.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter.

Facility has thermometers and test strips. PIC demonstrated proper food safety knowledge. Discussed obtaining ODH certified certificate through health department. Facility in need of cleaning and minor repairs. Facility does have employee health policy in place. All temperatures that were taken were good.
