3717-1-04.5(C) / Cooking and baking equipment - cleaning frequency
Observed that the dough machine bowl and paddle had not been used yet was covered in dried dough. Clean equipment at a sufficient frequency and after use to prevent drying of food debris unto utensils.

3717-1-06.1(M) / Outer openings - protected.
Observed that the backdoor to the facility did not have a door sweep. Purchase a door sweep for the rear door to prevent any rodents or pests from entering the facility.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Observed bag of onions stored directly on the floor. PIC moved these to a prep table at the time of inspection. Store foods at least 6 inches off of the ground to prevent contamination from splashing or spilling.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Observed that the can opener at the facility was covered in pizza sauce and debris (sits between trash and deli slicer). During the inspection the can opener was soaked and scrubbed by employees. Clean nonfood contact equipment thoroughly and often to prevent build up.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Observed dust in several areas of the kitchen including but not limited to: - Along shelves at pizza cutting table - Along shelves around the three compartment sink. - On fan guards in coolers - On the walls near the trash can Clean the facility thoroughly and often to prevent build up.

3717-1-06.1(A) / Floors, walls, and ceilings.
Observed that the facility had several areas with damaged including but not limited to: - Several browned ceiling tiles in the kitchen area indicative of a roof leak. Replace and repair leak. - Damaged floor tiles in the back of the kitchen around the dough bowl and walk in cooler - Damage around the floor drain near the three compartment sink.

3717-1-06.1(I) / Light bulbs - protective shielding.
Repeat Observed some light fixtures in the back had open shields that would not prevent glass from falling through. Properly shield lights so that glass cannot fall unto open foods or utensils below in case of one breaking.
