Donuts available for customer self service were not prominently labeled with the common name of the food and ingredient list. The labels shall be in plain view of the customer.
3717-1-04.5(A)(1)/Cleanliness of equipment food-contact surfaces and utensils.
Observed dark and mildew looking substances on the nozzles of the pop machine and on the tray that catches water. No corrective action was taken at the time of inspection.
3717-1-02.4(C)(14)/PIC: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
There was no employee health policy available at the time of inspection to show that employees are informed in a verifiable manner of their responsibility to report symptoms and illnesses as they relate to food borne diseases. Inspector gave PIC a copy of an appropriate employee health policy at the time of inspection.
3717-1-02.4(B)(11)/PIC: Demonstration of Knowledge - Explaining correct procedures for cleaning and sanitizing
Person in Charge was unable to demonstrate how to test the concentration of the sanitizer or what the correct concentration should be. Inspector discussed proper sanitizing and demonstrated testing with the employees.

Violations have been addressed. Observed employee health policy- kept on an on-line training system for each employee. Nozzles on pop machine are clean. Employee have been trained in how to test sanitizer concentration. Donuts have been labeled and ingredients list placed in view of the customer. Person in charge, Tim, has taken his level 2 servsafe course and passed. He is waiting on certificate from the State. Please print off and display certificate in the facility once obtained.
