[?] A summary of the violations found during the inspection are listed below.

3717-1-04(A) / Multiuse utensils and food contact surfaces - material characteristics.
Observed that the facility had rubbermaid containters for its foods that were not food grade. Food shall be stored in food grade containers to withstand day to day use, cleaning, and prevent imparting any odors or tastes to the foods.
To prevent contamination, materials that are used in the construction of utensils and food contact surfaces may not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions shall be: Safe; durable, corrosion-resistant, and nonabsorbent; Sufficient in weight and thickness to withstand repeated warewashing; finished to have a smooth, easily cleanable surface; and resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition.
3717-1-04.3(B) / Fixed equipment installation - spacing or sealing.
Observed a cutting board which had been affixed to the lid of the chest freezer. Fixed equipment shall be installed so that it can be easily cleaned underneath or properly sealed to prevent any dirt or debris from accumlating or seeping into this area.
Equipment that is fixed shall be installed so that it is spaced to allow access for cleaning, from adjoining equipment, walls, and ceilings a distance of not more than one thirty-second inch or one millimeter; or sealed to adjoining equipment or walls, if the equipment is exposed to spillage or seepage; Counter-mounted equipment shall be installed to allow cleaning of the equipment and areas underneath and around the equipment as specified in this rule.
3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Observed that the walk in cooler at the facility had a damaged seal. Ensure that seals are maintained in good repair to prevent any warm air from entering the cooler and causing damage to equipment or foods.
Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Observed that the facility had a large amount of unnecessary items in the back including an old ice machine full of items. Items not necessary for day to day operations shall be removed to prevent harborage for rodents or other pests.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter.

Comments
At the time of inspection the facility had made much progress to correct violations. violations not listed in this report have been corrected. At the time of inspection the foods were properly date marked, fans and surfaces were maintained clean, the facility had replaced towels used to cover shelves with rubber matting.

Did not observe employees washing their single use gloves. Employees shall properly replace gloves and wash hands when necessary.
