3717-1-02.4(B) / Person in charge: demonstration of knowledge.
Critical Person in charge did not indicate knowledge of proper warewashing and sanitization of food contact surfaces. Person in charge must take approved ODH level 1 or level 2 food safety training
To prevent or correct factors that may cause foodborne illness, based on the risks inherent to the food operation, during inspections and upon request, the person in charge shall demonstrate to the licensor the applicable food safety knowledge at the time of inspection.
3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Bag of sugar opened in storage room and not protected from contamination. Once opened, all food must be stored in a container to protect from contamination.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.
3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Observed several ready to eat sandwiches made from a commisary with expired use by dates. Discard all ready to eat TCS sandwiches or foods7 days after opening or when manufacturer use by date expires to prevent illness. Food was voluntarily discarded.
To prevent illness, refrigerated, ready-to-eat TCS food that has been date marked in the facility shall be discarded if it: (a) exceeds the temperature or time specified in (G)(1) of this rule, except the time the food is frozen; (b) is in a container or package that does not bear a date or day; or (c) is marked with a date or day that exceeds the time and temperature combination specified in (G)(1) of this rule.
3717-1-07.1(A) / Poisonous or toxic materials: Storage: separation.
Critical Bleach/chemicals stored on shelf above food. Store all chemicals below or saprate from food to protect food from contamination.
To prevent contamination, poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, or single-service or single-use articles by: 1) Separating the poisonous materials by spacing or partitioning; and 2) Locating the poisonous or toxic materials in an area that is not above food, equipment, utensils, linens, single-service articles or single-use articles. .

3717-1-04.6(A) / Food-contact surfaces and utensils - sanitization required
Sanitizer and test strips not available in warewashing area for proper wash-rinse-sanitizing of food contact surfaces. Obtain and use for proper sanitizing of food contact surfaces.
Equipment food-contact surfaces and utensils shall be sanitized.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Observed unecessary items in storage areas that prevents routine cleaning of the facility. Remove all unnecessary items from facility (tire, boxes, old light bulbs, etc.)
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter.
