3717-1-02.1(A) / Employee health: Reporting by food employees
Critical Corrected During Inspection Did not require employee(s) to report symptoms, diagnosed illness by a health care provider, or high risk situations as specified in rule. The license holder shall require food employees and conditional employees to report to the person in charge information about their health as it relates to diseases that are transmissible through food. Have each employee sign their own copy of the employee illness agreement and keep it on file.

3717-1-04.1(C) / Food-contact surfaces - cleanability
Critical Observed cutting boards with deep cuts in them that are harboring food debris and bacteria. Food contact surfaces shall remain smooth and easily cleanable. Have cutting boards resurfaced or replace with smooth cutting boards.

3717-1-02.4(C)(17) / PIC: duties - ensure the facility has written procedures for responding to vomiting and diarrheal events.
Corrected During Inspection No written procedures for responding to vomiting or diarrheal events observed in the facility. The RFE shall have written procedures for employees to follow when responding to vomiting or diarrheal events that involve discharge onto surfaces in the food service operation. Keep copy of procedure that was provided during inspection in biohazard kit.

3717-1-06.1(A) / Floors, walls, and ceilings.
Observed several tiles on floor that were broken and not smooth. Floors, walls, and/or ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. Have floor tiles replaced so that they are smooth and easily cleanable.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed food and debris all over floor and walls. Physical facilities shall be cleaned as often as necessary to keep them clean. Deep-clean facility so that food, dirt, and debris are not built up.

3717-1-06.1(I) / Light bulbs - protective shielding.
No protective shielding on light in kitchen area. Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food. Have a shield installed on light to ensure safety of food and employees.
