[?] A summary of the violations found during the inspection are listed below.

3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Observed goodness knows snack squares in middle of all non food items shelf. Observed lemon cut in sandwich cooler sitting on top with no wrapping.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.
3717-1-05.1(O)(2) / Using a handwashing sink - other uses prohibited.
Critical Observed hand sink next to 3 comp sink with dirty dishware in it.
To prevent contamination, a handwashing sink may not be used for purposes other than handwashing.

3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Observed person in charge knew no questions relating to food safety. PIC did not know where food license is, where employee health policy was, where vomit diarrhea spill kit is, unable to show where thermometer, and test strips were and unable to show certified serv safe certificate.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.
3717-1-03.2(D) / Food storage containers - identified with common name of food.
Observed sauces in cooler #1 with no label indicting what the sauce is. Observed what appears to be sugar, a white granule substance in shakers next to coffee grounds.
Working containers holding food or food ingredients that are removed from their original packages shall be identified with the common name of the food, except containers holding food that are readily and unmistakably recognized.
3717-1-04(I) / Nonfood-contact surfaces - materials.
Observed absorbant material under gatorade sitting at. Observed exposed wood above 3 comp sink where its absorbing water and growing mold. Observed counter tops with exposed top absorbant material being exposed.
Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be smooth and durable.
3717-1-06.2(E) / Handwashing signage.
Observed hand washing sign above 3 comp sink and not above hand sink.
A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees.
3717-1-06.4(A) / Repairing.
Observed front of pop cooler broken and on ground. Observed walls with chipping and scrapes specifically on back wall by door but other areas as well. Observed broken display case holding donuts. Display case holding donuts above microwave has case with tape on it with hair sticking to it. Observed missing baseboard behind toilet in corner causing hole. Hole potential opening for rodents. Observed front of countertop where register is being held with tape. Observed doors non self closing #2 door.
The physical facilities shall be maintained in good repair.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed crystallized mixes in cappuccino machine indicating it has been a while since cleaned. Observed fryer machine not used today with food particles in it. Observed ice build up around ice cream cooler. Observed splatters on wall above 3 comp sink. Observed mold growing on exposed wood above 3 comp sink. Observed dirt/build up on floors around isles, machines, and shelving. Observed splatters on door to beer cooler. Observed splatters on wall above milk cooler.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Observed hot holding unit on counter top broken and not being used. Observed a lot of clutter and disorganization by back door, corner by coolers, room in back, and 3 comp sink room. Could not walk in back area with all stuff. Re organize and the premise should be cleared of all unnecessary items and litter.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter.

No hot food being made at time of inspection. Facility needs to be deep cleaned under shelving, floors, between equipment, walls, celings, inside equipment. Facility needs repairs in several places. Back of facility needs organized. Owners are on vacation until the 27th of January. Facility needs ODH certified serv safe course level 2, vomit/diarrhea spill kit with procedure, test strips, thermometer, employee health policy and pest control.
