[?] A summary of the violations found during the inspection are listed below.

3717-1-02.2(D) / Hands and arms: where to wash.
Critical Observed 2 utility sinks with soap and paper towel dispensers indicating hand washing was being done at these sinks. Only hand washing sinks shall be used for handwashing.
To prevent contamination, food employees shall clean their hands in a handwashing sink or approved automatic handwashing facility and may not clean their hands in a sink used for food prep or warewashing, or in a curbed cleaning facility used for the disposal of mop water and similar liquid waste.
3717-1-02.4(C)(14) / Person in charge: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Observed facility does not have verifiable proof of employee health policy. FAcility shall obtain verifiable proof of health policy in place.
To prevent transmission of foodborne illness, the person in charge shall ensure that food and conditional employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food.
3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Observed sliced apples stored in fridge with no protective covering. All items stored shall be protected from contamination by covering.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.

3717-1-04.8(A) / Equipment and utensils - air-drying required.
Observed serving trays being stored or about to use to serve without being air dried (wet nesting).
After cleaning and sanitizing, equipment and utensils shall be air-dried or used after adequate draining before contact with food and utensils, and may not be cloth dried.
3717-1-06.1(M) / Outer openings - protected.
Observed light that can be seen under exterior front door.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.
3717-1-06.4(A) / Repairing.
Observed rough floors in several spots in kitchen. All floors, walls, and ceilings shall be smooth and easily cleanable. Observed broken hot water handle at sink on line. Observed continuous running water at sink on line unable to be turned off. Observed leaky faucet by fridges unable to turn off.
The physical facilities shall be maintained in good repair.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed ceiling corners in several spots beginning to grow mold. Observed food spillage in bottom of hot box unit.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
