Critical Person in Charge was unable to demonstrate how to test the concentration of the sanitizer or what the correct concentration should be. Inspector discussed proper sanitizing and demonstrated testing with the employees.

3717-1-02.4(C)(14) / PIC: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical There was no employee health policy available at the time of inspection to show that employees are informed in a verifiable manner of their responsibility to report symptoms and illnesses as they relate to food borne diseases. Inspector gave PIC a copy of an appropriate employee health policy at the time of inspection.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Observed dark and mildew looking substances on the nozzles of the pop machine and on the tray that catches water. No corrective action was taken at the time of inspection.

3717-1-02.4(A)(2) / PIC - Level Two Certified Manager
Facility does not have a person certified in level 2 food safety. Facility must have at least once person certified and an ODH certificate kept on site.

3717-1-03.5(C)(3) / Food labels - bulk food
Repeat Donuts available for customer self service were not prominently labeled with the common name of the food and ingredient list. The labels shall be in plain view of the customer.
