3717-1-02.2 (B) / Hands and arms: cleaning procedure
Critical Employee did not wash their hands for 20 seconds. Ensure all employees wash their hands for a full 20 seconds before rinsing and drying their hands.

3717-1-04.4(N)(1) / Manual and mechanical warewashing equipment, chemical sanitization (chlorine) - temp., pH, concentration, and hardness
Critical Repeat Chlorine sanitizer was at a lower concentration than specified on the sanitizer container. Ensure dishes and utensils are being sanitized at the appropriate concentration.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Dark streaks were found in the interior of the ice machine. Ensure that all food contact surfaces, including the interior of ice machine, are regularly washed, rinsed and sanitized.

3717-1-06.1(M) / Outer openings - protected.
Repeat Daylight was visible from the corner of the back door. Ensure all openings around the door frame are sealed to prevent intrusion by pests.

3717-1-04(I) / Nonfood-contact surfaces - materials.
Repeat Tape found on gaskets on doors to walk-in cooler and walk-in freezer in the back of the facility, as well as on the handle of the door to a reach-in cooler. Ensure all tape is removed to allow the equipment surfaces to be smooth and easily cleanable.

3717-1-06.4(A) / Repairing.
Repeat Walk-in freezer had excessive ice accumulation on floor near door. Ensure whatever is causing such ice accumulation is repaired or adjusted as needed to prevent the issue from recurring.
