[?] A summary of the violations found during the inspection are listed below.

3717-1-06.4(K) / Controlling pests.
Critical Observed mouse droppings in storage areas. Hire a licensed pest control operator to eliminate pests from the facility. Will follow up in 30 days to evaluate and ensure a proffessional has begun treatment. No household treatment chemicals may be used. Remove unnecesary items and clutter to prevent harborage conditions. No evidence of pests in food/food prep areas. If pests are observed in food areas, it is ground for immediate closure.
To prevent illness, the presence of insects, rodents, and other pests shall be controlled by: Routinely inspecting incoming shipments of food and supplies; routinely inspecting the premises for evidence of pests; Using methods, if pests are found, such as trapping devices or pesticides used according section (C), (K) and (L) of rule 3717-1-07.1 of this code; and eliminating harborage conditions.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Observed unnecessary items and clutter throughout the facility. Remove unused equipment, clutter, and other unnecessary items from the facility to prevent areas for mice and pests to hide and live.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter.

Complaint reported to our office regarding mice in the facility. Upon inspection, observed mouse droppings in back storage area. Hire a certified pest control operator to treat the facility for rodents.
Food areas are kept clean. Observed proper date marking and temperature holding of TCS foods.
Manager has taken Servsafe course; waiting on results.
This facility needs a spill kit and written procedure for cleaning up fecal and vomit accidents.
