3717-1-02.1(A) / Employee health: Reporting by food employees
Critical Corrected During Inspection Did not observe any form of an employee illness agreement in the facility and PIC could not provide one. Food license holders must require employee(s) to report symptoms, diagnosed illness by a health care provider, or high risk situations as specified in rule. Have current employees and all future employees sign the agreements that were provided during the inspection, and keep on file.

3717-1-05.1(O)(1) / Using a handwashing sink - accessible at all times
Critical Observed a handwashing sink in vegetable preparation area that was completely blocked by materials that did not allow access by workers. Handwashing sinks shall be maintained so that they are accessible at all times for employee use. Ensure all handwashing sinks remain clear so that employees can properly wash their hands. PIC attempted to clear the area, but it still remained in a state of clutter that would not allow use by employees.

3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Observed raw steak being stored above cooked ham in open meat cooler. Food shall be protected from cross contamination by separating raw animal foods during storage from cooked RTE food. Ensure that raw animal food is stored below cooked food. Raw animal food was voluntarily rearranged so that it was stored below cooked ham.

3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Observed a box of meat scraps containing different types of raw meat that included raw ground beef and pieces of chicken placed on top of boxes of raw whole pork in walk-in meat cooler. Food shall be protected from cross contamination by separating raw animal foods in accordance with the code. Ensure food is organized in such a way that cross contamination will not occur. Box of meat scraps was voluntarily moved to a safer spot in the cooler.

3717-1-02.4(C)(17) / PIC: duties - ensure the facility has written procedures for responding to vomiting and diarrheal events.
Corrected During Inspection Did not observe written procedures for responding to vomiting or diarrheal events. The FSO or RFE shall have written procedures for employees to follow when responding to vomiting or diarrheal events that involve discharge onto surfaces in the establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter. Keep the written procedure provided during the inspection with the kit that was constructed on site by the PIC in case of a biohazard event.

3717-1-05.1(C)(1) / Handwashing sink - required water temperature
Observed hand washing sinks in bathrooms with water below 100°F. A hand washing sink shall be equipped to provide water at a temperature of at least one hundred degrees F through a mixing valve or combination faucet. Have sinks repaired so that they provide water at a temperature of at least one hundred degrees F.

3717-1-04.1(Y) / Temperature measuring devices.
Did not observe any form of a working thermometer in two walk-in coolers. In a mechanically refrigerated or hot food storage unit, the sensor of a temperature measuring device shall be located to measure the air temperature or a simulated product temperature in the warmest part of a mechanically refrigerated unit and in the coolest part of a hot food storage unit. Place a thermometer in the warmest part of each cold holding unit. Verified that the coolers were maintaining a temperature of forty-one degrees F or below with personal thermometer.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Repeat Observed food including raw animal meat being stored directly on the floor in the walk-in freezer. Food shall be protected from contamination by storing the food at least six inches above the floor. Place the food on risers or otherwise raise the food at least six inches above the floor in all areas to prevent contamination.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Observed broken gaskets on three different doors on milk and dairy cooler. Equipment components must be kept intact, tight or properly adjusted. Repair the broken gaskets on the doors of the cooler.

3717-1-06.4(F) / Drying mops.
Mops dried improperly. After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies. After use, ensure each mop is hung to dry to prevent build-up of bacteria.

3717-1-06.4(A) / Repairing.
Observed ice building up on the fans and around the corners of the room in two walk in freezers. Observed boxes underneath ice that were not voluntarily moved upon request and are therefore at risk of contamination. All physical facilities shall be maintained in good repair. Have fans serviced so that ice does not form behind them or around the freezers.

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Observed a build-up of dust on vents of fan in two walk-in freezers. Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination by dust, dirt, and other materials. Clean vents so that they do not contaminate the food in the freezers.

3717-1-06.2(I)(1) / Lighting - intensity (10 FC)
Observed light burnt out in walk-in freezer. Light intensity must be at least ten foot candles in walk-in refrigeration units. Replace burnt-out light bulb to provide adequate lighting in required areas.
