3717-1-07(B) / Poisonous or toxic materials: Working containers - common name.
Critical Repeat Corrected During Inspection Observed a spray bottle of sanitizer that was not properly labeled. The manager labeled this bottle at the time of inspection.
To prevent health hazards, working containers used for poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material.

3717-1-04(A) / Multiuse utensils and food contact surfaces - material characteristics.
Observed knives at the facility that were not maintained sharp. There were some large pitting in the sub knife and the bread knife. Knives shall be maintained to prevent them from chipping or pitting into foods.
To prevent contamination, materials that are used in the construction of utensils and food contact surfaces may not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions shall be: Safe; durable, corrosion-resistant, and nonabsorbent; Sufficient in weight and thickness to withstand repeated warewashing; finished to have a smooth, easily cleanable surface; and resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Observed that the pizza prep shelves had been cleaned. However the shelving for clean utensils, along the walls above prep tables, etc had not been properly cleaned. CLEAN SHELVING THOROUGHLY AND OFTEN TO PREVENT DUST BUILD UP WHICH MAY FALL INTO FOODS BELOW.
Nonfood-contact surfaces of equipment shall be kept clean.
3717-1-05.4(F)(1) / Outside receptacles - tight fitting lids
Repeat Observed that the dumpster lid had not been replaced. CONTACT THE DUMPSTER COMPANY TO HAVE THE LIDS REPLACED THIS WILL PREVENT ANY RODENTS OR OTHER PESTS FROM ENTERING THE FACILITY.
Receptacles and waste handling units for refuse, recyclables, or returnables used outside with materials containing food residue, shall be designed and constructed to have tight-fitting lids, doors, or covers.
3717-1-06.1(A) / Floors, walls, and ceilings.
Repeat Observed areas of the floors and walls that were not smooth and cleanable such as: -Behind the mop sink -Around kitchen floor drains -Browned ceiling tiles. PREMISES SHALL BE MAINTAINED IN GOOD REPAIR.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Observed a build up of food debris underneath shelving near the service counter and under equipment. CLEAN BEHIND AND UNDER EQUIPMENT THOROUGHLY AND OFTEN TO PREVENT FOOD DEBRIS BUILD UP.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.

At the time of inspection there were repeated violations regarding the cleanliness of the facility. The inspector spoke with the owner during the inspection. Per the owner he intends to replace racks around the facility. The owner has a cleaning list for his staff that was shown to the inspector. At a glance the list appears to have good cleaning frequencies for the facility to promote good hygiene/cleanliness. Ensure that staff are properly following the procedures.
