Repeat Facility does not have an employee with manager certification in food protection. Ensure at least one employee in a Risk Level III facility has manager certification.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Gaskets of doors to coolers containing Mike's Hard Lemonade, Blue Moon, and Bud Light products were damaged and need to be repaired or replaced. Ensure door gaskets are maintained intact to ensure an appropriate seal between door and cooler.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Excessive frost accumulation found in ice cream freezer. Ensure non-food contact surfaces are maintained clean to sight and touch.
