III / Preventing Contamination by Hand
Observed utensils dispensed with the mouth contact portion and handles intermixed. Store silverware with handles facing the same direction to prevent bare hand contact with the portion a customer will put into their mouth.

IV / Demonstration of Knowledge
Observed valid serv safe certificates but no ODH approval paperwork. Inspector will provide location with link to ODH reciprocity form.

VII / Protection from Contamination
Observed that the facility was not allowing dishes or cutting boards to area dry before stacking causing some mold/mildew growth. Allow surfaces to fully dry before stacking as this will lock the water in place.

X / Chemical
Observed a chemical bottle that was hung above the three compartment sink and unlabeled. All working containers of chemicals shall be A. properly labeled and B. stored in a designated location away from food preparation or clean dishes. PIC labeld and moved this at the time of inspection. Observed a hot shot pest strip hanging in the kitchen. Hot shot pest strips have instructions that state they are not for use in food preparation areas of the facility. Furthermore pesticides shall be applied by a licensed applicator. PIC removed pest strip. Use all chemicals to law and label.
