Critical observed dirt and debri on slicer. Food contact items must be washed, rinsed, and sanitized to prevent bacteria growth.
To prevent contamination, food shall only contact the surfaces of cleaned and sanitized equipment and utensils, linens or single-use and/or single-service articles.
3717-1-05.1(D) / Backflow prevention - air gap.
Critical Prep sinks must have an indirect plumbing connection. An air gap is required.
To prevent contamination, an air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment, or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than one inch (25 millimeters).
3717-1-05.1(O)(1) / Using a handwashing sink - accessible at all times
Critical Observed dirt/ddebri in hand wash sinks- hand was sinks shall be clean and ready for use.
To ensure frequent and proper handwashing, a handwashing sink shall be maintained so that it is accessible at all times for employee use.

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Observed unlabeled working container-oil? All working containers of food removed from original container that is not easily identifiable must be labeled with common name.
Working containers holding food or food ingredients that are removed from their original packages shall be identified with the common name of the food, except containers holding food that are readily and unmistakably recognized.

observed no sanitizer test kit available.
To ensure proper sanitization, the concentration of the sanitizing solution shall be accurately determined by using a test kit or other device.

Observed dirt and debri build up on floors and walls throughout facility- observed dirt and debri on door handles- sinks-sides of equipment.
Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

Repeat 11-20-17. observed dirt/food debri on outside of slicer, dough mixer, shelving of coolers and floors of coolers.

observed grease trap that appears to be too small. Replace with proper size- contact building department 740-477-8282 re: any permit needed.
If used, a grease trap shall be located to be easily accessible for cleaning.
3717-1-06.2(I)(3) / Lighting - intensity (50)
Repeat 11-20-17. Observed low light intensity at slicer.
The light intensity shall be at least fifty foot candles (five hundred forty lux) at a surface where a food employee is working with food, utensils or equipment such as knives, slicers, grinders, or saws where employee safety is a factor.

observed burnt out lights- All light fixtures must be in good repair to ensure tlight intensity in prep areas meet minimum requirement- 50 ft. candles at surfacces where a food employee is working with food, slicers, knives.
