| Standard inspection completed for change of ownership of facility to Underdogs LLC. Plan review submitted on September 15 2015 and approved. The following was noted during the inspection: 1. Building department completed inspection of upgrades on October 7, 2015. 2. Fire department inspection completed on September 15, 2015. 3. Facility equipment cleaned. New equipment installed. See plan review for equipment details. 4. New floor installed as well as base coving. Floor is approved and is easily cleanable and non skid. 5. Plumbing upgrades completed, food prep sink added with proper air gap, new 3 comp. sink installed with a new faucet and spray hose. 6. Walls and ceiling painted. No violations noted during the inspection. License transferred to new ownership. Ok to open. |
