Critical Quaternary ammonium sanitizing solution at incorrect concentration. Observed sanitizer reading above 400ppm. Quat sanitizer must read between 200-400ppm. Dilute solution so that concentration isnt so high.

Critical Presence of live insects, rodents, and other pests. Observed gnats in display case and around drive thru. Control gnats in the store.
3717-1-03.2(M) / Wiping cloths - use limitation.
Improper use and/or maintenance of wiping cloths. Observed wiping cloths in sanitizer buckets stored on floor. Sanitizer buckets must be stored six inches off of ground.

Non-food contact surface(s) not cleaned at the required frequency. Observed rinse compartment of three compartment sink with build up of debris. Clean three compartment sink.
