[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(F)(1) / Time/temperature controlled for safety food - hot and cold holding.
Critical Observed ham at 48F and raw ground beef at 47F in the prep cooler. Other foods in the prep cooler were held at the correct temperature of 41F or below. It is likely that temperature abuse is occur due to being in and out of the cooler. Cold held TCS foods shall be held at 41F or below to prevent bacterial growth.
To prevent the growth of pathogens, except during preparation, cooking, or cooling, or when time is used as a public health control, TCS food shall be held at 135°F or above (except that roasts cooked using time and temperature parameters in 3717-1-03.3 may be held at 130° F), or at 41°F or less.

3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Observed that the facility did not have proof of state approved level 2 food safety training on site, despite prior inspection stating that it should be held on site. Facility must have proof of level 2 food safety training available for inspectors.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.
3717-1-02.4(C)(16) / Person in charge: duties - ensure the facility has written procedures for vomiting and diarrheal events.
Observed facility did not have a vomit or bodily fluid clean up procedure. A vomit or bodily fluid clean up procedure shall be available.
Person in charge shall ensure that the FSO or RFE have written procedures for employees to follow when responding to vomiting or diarrheal events that involved discharge onto surfaces.
3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Observed the open door freezer seal was tearing. All gaskets and seals shall be maintained in good repair to prevent warm air from entering the cooler and causing damage to foods or equipment.
Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
3717-1-05.4(H) / Toilet room receptacle - covered.
Observed no covered receptacle in unisex restroom. Restrooms shall have a covered receptacle for hygiene products.
A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
3717-1-06.1(M) / Outer openings - protected.
Observed light visible underneath the entrance to the kitchen. Install a door sweep or weather stripping to prevent any insects or pests from entering the facility.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.
