Critical Repeat Observed handsink in produce department was blocked. Code states all handwashing sinks must be accessible at all times. Make sure nothing is blocking hand sink.

3717-1-02.4(C)(17) / PIC: duties - ensure the facility has written procedures for responding to vomiting and diarrheal events.
Repeat No written procedures for responding to vomiting or diarrheal events. Code states there must be a written procedures for responding to biohazard events. Make sure you have a procedure in place.

3717-1-03.4(A) / Frozen food - temperature and time control.
Observed frozen foods being stored above the safe store line in two middle freezers on floor, product starting to thaw. Code states frozen foods must be kept frozen. Make sure frozen foods are stored below the red safe load line on freezer to prevent thawing.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Repeat Observed boxes of food being stored on floor of dairy cooler as well as the freezer. Code states food must be stored at least 6 inches off of the ground to prevent contamination. Make sure food is stored 6 inches off of the ground.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Observed gaskets on milk coolers not in tact. Code states gaskets on doors must be in tact and maintained. Replace gaskets on milk cooler doors with broken gaskets.

Observed ice buildup in walk in freezer with food stored underneath. Code states equipment must be maintained. Make sure there is no significant ice buildup in freezer.

Observed hood above stove in backery dusty. Code states non contact food surfaces must be cleaned with a frequency that prohibits contamination of food by debris. Make sure hood is dust free.

Repeat Observed mops drying improperly. Code states mops must be hung up to dry to prevent bacterial growth. Make sure mops are hung up to dry.

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Repeat Observed fans in meat cooler dusty. Code states air ventilation units must be cleaned at a frequency that prohibits contamination of food by dust and debris. Make sure fans are cleaned and dust free.
