[?] A summary of the violations found during the inspection are listed below.

3717-1-02.2(C) / Hands and arms: when to wash
Critical Observed employees at the facility who were changing tasks and donning gloves without washing hands. Observed multiple employees wash their gloves. Washing gloves is not an acceptable practice. Employees shall properly remove single use gloves and wash hands when necessary such as when changing tasks or when gloves have become contaminated.
To prevent contamination, food employees shall clean their hands and exposed portions of their arms after switching between working with raw food and with ready to eat food; before donning gloves to initiate a task that involves working with food; and after other activities that contaminate the hands.
3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Observed multiple containers of tomatoes which had exceeded 7 days. Ready to eat, TCS foods that exceed 7 days shall be discarded.
To prevent illness, refrigerated, ready-to-eat TCS food that has been date marked in the facility shall be discarded if it: (a) exceeds the temperature or time specified in (G)(1) of this rule, except the time the food is frozen; (b) is in a container or package that does not bear a date or day; or (c) is marked with a date or day that exceeds the time and temperature combination specified in (G)(1) of this rule.
3717-1-07(B) / Poisonous or toxic materials: Working containers - common name.
Critical Corrected During Inspection Observed an unlabeled spray bottle of delimer. At the time of inspection the manager properly labeled the chemical bottle. All chemical bottles shall be properly labeld to prevent them from accidental misuse.
To prevent health hazards, working containers used for poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Observed that the facility had a bag of onions stored directly on the floor. Store foods at least 6 inches off of the floor to prevent any contamination from splash or spilling.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor.
3717-1-03.2(K) / In-use utensils - between-use storage.
Observed that the facility had some scoops with damaged handles or with their handles below the surface of the ingredients (Salt, sugar, etc.) In use utensils shall have handles to minimize bare hand contact with the ingredient.
During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored as described in this rule.
3717-1-04(A) / Multiuse utensils and food contact surfaces - material characteristics.
Observed that the facility had rubbermaid containters for its foods that were not food grade. Food shall be stored in food grade containers to withstand day to day use, cleaning, and prevent imparting any odors or tastes to the foods.
To prevent contamination, materials that are used in the construction of utensils and food contact surfaces may not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions shall be: Safe; durable, corrosion-resistant, and nonabsorbent; Sufficient in weight and thickness to withstand repeated warewashing; finished to have a smooth, easily cleanable surface; and resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition.
3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Observed facility lining shelves with paper towels or cardboard. Paper towels and cardboard are absorbent materials and may collect food debris or attract pests. Nonfood contact surfaces shall be water resistant and easily cleanable such as rubber matting.
Nonfood-contact surfaces shall be designed and constructed to allow easy cleaning and to facilitate maintenance.
3717-1-04.3(B) / Fixed equipment installation - spacing or sealing.
Observed a cutting board which had been affixed to the lid of the chest freezer. Fixed equipment shall be installed so that it can be easily cleaned underneath or properly sealed to prevent any dirt or debris from accumlating or seeping into this area.
Equipment that is fixed shall be installed so that it is spaced to allow access for cleaning, from adjoining equipment, walls, and ceilings a distance of not more than one thirty-second inch or one millimeter; or sealed to adjoining equipment or walls, if the equipment is exposed to spillage or seepage; Counter-mounted equipment shall be installed to allow cleaning of the equipment and areas underneath and around the equipment as specified in this rule.
3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Observed that the walk in cooler seal at the facility had duct tape holding the seal on the door. Maintain cooler seals in good repair to prevent any warm air from entering the cooler and damaging the equipment or foods.
Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed a build up of dust and debris on the standing fans in the facility. Clean fans thoroughly and often to prevent dust from being blown through the facility. Observed dirt and debris on the shelves in the walk in cooler. Clean the shelving to prevent dust from falling into foods in the cooler.
Nonfood-contact surfaces of equipment shall be kept clean.
3717-1-06.1(M) / Outer openings - protected.
Observed that the facility had a magnetized screen for the rear door that had large gaps. Outer openings shall be properly protected with a screen door or other method that prevents insects, rodents, or other pests from entering the facility.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.
3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Observed dust building up along the inside of the hood system. Clean the hood system often to prevent any dust from contaminating foods or food contact utensils underneath.
Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination. If vented to the outside, ventilation systems may not create a public health hazard or nuisance or unlawful discharge.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Observed that the facility had a large amount of unnecessary items in the back including an old ice machine full of items. Items not necessary for day to day operations shall be removed to prevent harborage for rodents or other pests.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter.
