Critical Spray hose extends below the rim of the disposal sink.
Maintain spray hose so hose has sufficient air gap to protect water supply from backflow/contamination. .

3717-1-05.1(S)(1) / Plumbing system - maintained in good repair.
Repeat Wash sink on 3 compartment sink leaking water. Repair. Observed several faucets in kitchen leaking water and causing water to accumulate on the floor.
Repair leaks to prevent water on floor, slip hazards, and damage to floors.

Rice - cooking190F, chicken - hot hold - 191 F Pinnapple - cold hold 39F. Gloves, thermometers in use. Sanitizer observed at 200 ppm. Dish tank observed reaching proper sanitizing temperature. Prep areas kept clean. Food labelled and date marked.

Operation has been maintaining floors. Continue to maintain and repair floors to provide a smooth and cleanable surface that is also non-skid, long term plan to resurface of floor may be necessary to permanently repair floor.
