Critical Observed a build up of what appears to be mold on the inside of slushie nozzles. Clean and sanitize these areas daily to prevent accumulation.
Equipment food-contact surfaces and utensils shall be cleaned at a frequency specified in this rule.
3717-1-03.2(T) / Food preparation - preventing contamination from the premises
Observed food being prepared at the three compartment sink. Food prep shall be completed at the food prep sink to prevent contamination of food from dirty dishes and other sources of contamination.
During preparation, unpackaged food shall be protected from environmental sources of contamination.

Repeat The physical facilities are not being maintained in good repair. Wall by mop sink not adequately repaired. Seal hole to ensure pests cannot enter the facility.
