[?] A summary of the violations found during the inspection are listed below.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Observed several items on floors in walk in cooler and walk in ice freezer. Store food items six inches off the floor to prevent from contamination from the premises.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Walk in doors arenot self closing. Repair closing-arm on doors to ensure they close tightly.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings were not smooth and easily cleanable. Observed several broken, cracked,missing floor tiles through out the facility. Repair and replace to prevent accumulation of debris, potential for mold and mildew growth, and attraction of pests. Correct before next inspection
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. Correct By: 11-Jun-2016
3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Observedseveral ares in the facility where coving is missing or damaged. Replace coving so all surfaces are smooth, non absorbent, and easiliy cleanable.
In FSO's or RFE's in which cleaning methods other than water flushing are used for cleaning floors, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch; where water flush cleaning methods are used, shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and sealed.
3717-1-06.4(A) / Repairing.
Observed several holes in walls, paint chipping, andgrout missing in countertops. Maintain facilities to ensuresurfaces are easily cleanable and to prevent harborage of pests.
The physical facilities shall be maintained in good repair.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris in hard to reach areas, corners, and other areas through out the facility. More detailed cleaning is necessary to prevent the accumulation of debris.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Correct By: 18-Jan-2016
