[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed hard boiled eggs in dry storage while employees were sorting food. Eggs temped at 56 degrees. All cold holding must maintain at 41 degrees or below. Eggs were voluntarily discarded.

3717-1-06.4(K) / Controlling pests.
Critical Presence of live insects. Observed some flies and other small flying insects in kitchen. Facility must be able to properly control pests. Recommend fixing insect zap light, repair air blower at back door, and spraying consitantly for pests.

3717-1-05.1(L) / Handwashing sinks - location and placement.
Handwashing sink(s) are not conveniently located as specified in rule. Code states there is to be a handsink anywhere there is food prep and warewashing. Currently there is one handsink in kitchen. Replace handsink that was removed from kitchen. Submit a plan to the health department as to where you will be placing sink within 30 days. Replace or install new handsink in kitchen within 60 days.

Physical facilities not maintained in good repair. Floors throughout kitchen were observed with large holes that hold water. These spots included by the back door, the room with the 3 compartment warewash sink, and the dish room attached to the cafeteria. Repair holes in floor so that they can no longer hold water.

3717-1-06.3 (C)(1) / Employee accommodations - designated areas
Throughout facility, observed employee drinks in areas were food prep was being conducted or stroed above clean dishes. Also observed employee hoodie hanging on clean dish rack. Make sure all employee personal items are stored in a designated space away from food prep and clean dishes.

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Observed fan being used in 3 compartment sink warewash room with significant build up of dust. Make sure all fans are kept clean to avoid dust discharge in kitchen.
