[?] A summary of the violations found during the inspection are listed below.

3717-1-06.4(K) / Controlling pests.
Critical Observed the presence of flies in kitchen serving area and in dishwash areas. Increase pest control and prevention measures to prevent fly problems in kitchen.
To prevent illness, the presence of insects, rodents, and other pests shall be controlled by: Routinely inspecting incoming shipments of food and supplies; routinely inspecting the premises for evidence of pests; Using methods, if pests are found, such as trapping devices or pesticides used according section (C), (K) and (L) of rule 3717-1-07.1 of this code; and eliminating harborage conditions.

3717-1-06.4(A) / Repairing.
Observed several areas near dish tank where walls have holes or are damaged which may allow harborage of pests. ALso observed holes in ceiling in dish 3 sink room that also need enclosed.
The physical facilities shall be maintained in good repair. Enclose gaps or holes in walls to help reduce harborage areas for possible pests.

Cold held food sampled observed held below 41F. Sanitizer in use in proper concentration between 150 and 400 ppm. Gloves and hair restraints in use. Prep areas and food equipment kept clean. Food thermometers in use. The following corrections have been made since last inspection. 1. Dish tank sanitizing at 160F and in proper temperature. 2. Hand sink repaired near the line prep area / large kettle for use 3. Oven doors replaced on oven that was missing door. 4. spray hoses in dish room in good repair, no leaks. 5. Sanitizer in proper concentration. 6. Floor repaired near steam kettles and is smooth, durable, and cleanable.

Investigated complaint during inspection. Report was made to our office that maggots were observed in food. During inspection, did not observe food with signs of insect contamination. Observed foods protected from contamination by covering to help prevent contamination. Did observe flies in serving area which was discussed during inspection that increased pest control and prevention measures may be necessary to eliminate flies from serving area.

Serving line currently not in use during inspection due to problems with steam wells and water damage to steam wells and components from gaps / leaks in steam table. A temporary serving line is set up for use until the existing serving line is repaired or replaced. Discussed ensure plan reviews are submitted to our office if any extensive alterations are make to the facility such as moving location of sinks, installing new serving lines, and other alterations to existing facilities.

Observed several dining tables that are worn, pitted, and the cleanable non absorbent surfaces are worn, resulting in tables being difficult to clean. Suggest replacing or resurfacing any damaged wood table with a surface that is easy cleanable and non absorbent.

License is a risk level 3, and will be upgraded to a risk level 4 FSO during next license renewal period due to operation cooling and reheating food routinely.
