[?] A summary of the violations found during the inspection are listed below.

3717-1-04(I) / Nonfood-contact surfaces - materials.
Observed cardboard on shelving in several areas (Under milk in ice cream service area; shelving for whipped cream in the walk in cooler) please remove all cardboard surfaces from the facility to ensure all surfaces are non-absorbent, smooth, and easily cleanable.
Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be smooth and durable.
3717-1-04.1(KK) / Food equipment - certification and classification.
Observed microwave in customer self-service area not approved for commercial use. Replace microwave with one certified for commercial use by a recognized testing agency.
Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency.
3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Observed some build up on non food contact surfaces. Some milk spills/splashes in walk in cooler; front of slushie machine accumulating debris and what appears to be mold. Clean NFC surfaces more regularly to prevent such build up
Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

Comments
Observed proper hot/cold holding temperatures. Gloves, thermometers, test strips available. As of March 1, 2016 all facilities must have a spill kit and written procedure for cleaning up fecal and vomit spills. Effective March 1, 2017 all facilities must have one employee certified in level 2 food protection (Servsafe satisfies this requirement and is offered by our office)
