Critical Corrected During Inspection Food not properly protected from contamination by separation, packaging, and segregation. Observed containers of raw chicken and sliced potatoes that were uncovered inside of the cooler next to the fryers. PIC placed lids on containers at the time of inspection.

Critical Corrected During Inspection Food not properly protected from contamination by separation, packaging, and segregation. Observed an uncovered prepared pizza inside of the walk-in cooler. PIC covered pizza at the time of inspection.

3717-1-04.1(Y) / Temperature measuring devices.
Corrected During Inspection Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable. Observed the thermometer placed in the back of the chicken and coleslaw cooler. PIC moved thermometer to the front of the cooler at the time of inspection.

Food employee(s) not wearing a hair restraint. Discussed wearing hats/headbands/hairnets and beard nets to restrain facial hair. Correct by: 11/2/22.

Corrected During Inspection Clean equipment and utensils not stored in a self-draining position and covered or inverted. Clean personal pizza pans were not covered or inverted to protect from contamination. PIC inverted pans at the time of inspection.

Fixed equipment not properly sealed or spaced for cleaning. Observed the caulking around the hand sink was cracked. Correct by next inspection.

Equipment components are not intact, tight or properly adjusted. Observed torn gaskets on the doors of the pizza make table cooler. Correct by next inspection.

Equipment components are not intact, tight or properly adjusted. Observed the walk-in cooler door not shutting tightly and a loose gasket on the door of the walk-in cooler next to the office. Correct by next standard inspection.

Critical Insufficient air gap between the flood rim and the water supply inlet. Observed a less than one inch air gap underneath the prep sink. Correct by: 11/2/22.

Repeat Outdoor refuse storage surface not properly constructed and sloped to drain. Observed the dumpster and grease container setting in the grass. Correct by next inspection.

Physical facilities not maintained in good repair. Light shield inside the walk-in pizza cooler is cracked. Correct by next standard inspection.

Facility not maintained clean. Observed the floor under the shelf in the mop closet in need of greater cleaning frequency. Correct by: 11/2/22.
