Critical Corrected During Inspection Employees are not informed in a verifiable manner of their responsibility to report information about their health. Employee was unable to locate signed copies of the employee health policy. Employee had a blank copy to have employees read and sign.

Critical Corrected During Inspection Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed open packages of hot dogs, sausages and taquitos not dated. PIC verified open date and properly date marked. Reviewed proper date marking procedures.

Critical Corrected During Inspection Improper display of poisonous or toxic materials offered for retail sale. Observed eye drops stored above beverages on sale floor. PIC pulled to be relocated separated from food items.

Ingredient/allergen information was broken down per product component. Facility has a person with a ServSafe Manager Certification in Food Protection, however facility did not have a person with the Ohio Department of Health (ODH) Manager Certification in Food Protection. Ensure the ODH certification is obtained for next standard inspection.

Facility is heating and hot holding hot dogs, sausage, and taquitos. Facility will be moved to a risk level 3.
