Critical Corrected During Inspection Food not properly protected from contamination by separation, packaging, and segregation. Observed raw shelled eggs over ready to eat foods in the walking cooler. PIC moved eggs to acceptable location at time of inspection.

Critical Corrected During Inspection Food not properly protected from contamination by separation, packaging, and segregation. Observed dog treats on the microwave by the drive through above single use and food items. Items for nonhuman consumption should not be placed above or on any food/equipment/contact surface. PIC had dog treats moved to floor at time of inspection.

Critical Equipment food-contact surfaces or utensils are unclean. Observed blade of can opener to have visible residue. PIC placed can opener in the warewashing area to be cleaned at time of the inspection.

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Repeat Corrected During Inspection Working food containers not properly labeled. Observed unlabeled caramel on the bottom shelf of the topping table. PIC labeled the container at time of the inspection.

3717-1-03.2(Y) / Miscellaneous sources of contamination.
Corrected During Inspection Food not protected from contamination. Observed personal items in the cooler, and the back prep table. PIC had both moved from the areas of concern at the time of inspection.

Corrected During Inspection Nonfood-contact surfaces not easily cleanable. Observed a cone holder broken that renders the surface no longer smooth and easily cleanble. PIC decommissioned cone holder at time of inspection.

3717-1-04.4(A)(3) / Equipment - cutting or piercing parts of can opener not maintained.
Corrected During Inspection Can opener blade dull or creating metal fragments. Observed the can opener to be dull and have metal shavings. Replace by 11/09/2022.

Critical Corrected During Inspection Food contact surfaces not easily cleanable. Observed funnel with crack in the tip that renders the surface no longer smooth and easily cleanable. PIC decommissioned the funnel at time of inspection.

Non-food contact surface(s) not cleaned at the required frequency. Observed visible residue on the retail cake freezer shelving. Clean by 11/03/2022.

Repeat Physical facilities not maintained in good repair. Observed floor in several areas to begin deteriorating so the surface is no longer smooth and easily cleanable. Repair by next inspection.

Comments
Conducted and reviewed with Olivia (PIC), Brett (Owner), and Jade Sweitzer (EHSIT). Discussed proper placement for thermometer in both make tables. Thermometers should be placed in the warmest part of the cooler to get an accurate ambient reading. Also discussed one whisk in a bucket with the handle upward, explained that all handles should be facing in the same direction to avoid contamination. PIC completed ServeSafe and is going to submit to the state for the ODH certificate.
