3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Observed improper storage of food items. OBSERVED FOOD ITEMS/CONTAINERS SITTING DIRECTLY ON FLOOR IN CARRY OUT, AND ON FLOOR IN DRIVE-THRU.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor. STORE/STACK FOOD ITEMS/FOOD CONTAINERS OFF THE FLOOR TO PROTECT FROM CONTAMINATION. Correct By: 24-Apr-2015
3717-1-04.1(Y) / Temperature measuring devices.
Cold or hot holding equipment did not contain a thermometer, or the thermometer was not correctly placed. DID NOT OBSERVE THERMOMETERS IN ANY OF THE CUSTOMER SELF-SERVICE, REACH-IN BEVERAGE COOLERS , OR IN THE WALK-IN COOLER.
A properly designed temperature measuring device as specified in this rule shall be located in the warmest part of a cooling unit or the coolest part of a food warming unit. PROVIDE WORKING THERMOMETERS IN ALL OF YOUR COLD HOLDING UNITS TO MONITOR TEMP.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are dirty. OBSERVED DUST ACCUMULATION ON EXHAUST FAN GRATES IN WALK-IN COOLER AND ON VENTILATION GRATES IN CARRY-OUT AREA ABOVE FOOD ITEMS.
Nonfood-contact surfaces of equipment shall be kept clean. clean DUST ACCUMULATION FROM FAN GRATES IN WALK-IN COOLER AND FROM VENTILATION GRATES IN CARRY-OUT AREA TO PREVENT CONTAMINATION OF FOOD CONTACT SURFACES. Correct By: 24-Apr-2015

The light intensity inside equipment was less than twenty foot candles. OBSERVED SEVERAL OF THE CUSTOMER SELF-SERVICE BEVERAGE COOLERS WITH BURNT OUT, NON-WORKING LIGHT BULBS INSIDE UNITS.
The light intensity shall be at least twenty foot candles (two hundred fifteen lux): at a surface where food is provided for customer self-service such as buffets or salad bars or where fresh produce or packaged foods are sold or offered for human consumption; inside equipment such as reach-in and under-counter refrigerators and at a distance of thirty inches (seventy-five centimeters) above the floor in toilet rooms and in areas used for hand washing, warewashing, equipment storage, or utensil storage. REPLACE/REPAIR LIGHT BULBS INSIDE SEVERAL OF CUSTOMER SELF-SERVICE BEVERAGE REACH-IN COOLERS TO PROVIDE ADEQUATE LIGHTING. Correct By: 24-Apr-2015

The physical facilities are not being maintained in good repair. OBSERVED MANY CEILING TILES STAINED AND/OR SOILED THROUGHOUT THE FACILITY.
The physical facilities shall be maintained in good repair. REPLACE STAINED/SOILED AND SAGGING CEILING TILES THROUGHOUT FACILITY TO PROTECT FOOD FROM CONTAMINATION. Correct By: 24-Apr-2015
