Facility does not have an employee with manager certification in food protection. Manager certificate expired 04/11/2022. Facility needs manager with approved manager certificate by next standard inspection.

3717-1-02.4(C)(15) / PIC: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Corrected During Inspection Employees are not informed in a verifiable manner of their responsibility to report information about their health. Employee present in kitchen at time of inspection did not have signed illness agreement at time of inspection. PIC had employee read and sign at time of inspection.

3717-1-05.1(O)(1) / Using a handwashing sink - accessible at all times
Critical Corrected During Inspection Handwashing sink not accessible. Hand sink in room where food was being plated for service was not operational. PIC had serviced and brought into operation.

3717-1-03.7(D) / Highly succeptible population - pasteurized eggs or egg products
Critical Corrected During Inspection Pasteurized eggs not being used when required for a highly susceptible population. Facility serves preschool age children and has unpasturized shell eggs that are not used for baking or combined for one consumers serving at a single meal and served immediately. PIC removed raw shell eggs from service and is using pasteurized products moving foreword.

3717-1-07(B) / Poisonous or toxic materials: Working containers - common name.
Critical Corrected During Inspection Working containers of poisonous or toxic materials not properly labeled. Spray bottle containing sanitizer was not labeled with common name. PIC labeled.

3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Re-use of single-service or single-use articles. Single use water gallons reused for water storage. Facility is to discontinue the reuse of single use containers by next standard inspection.

3717-1-06.2(I)(1) / Lighting - intensity (10 FC)
Light intensity less than ten foot candles in required areas. Lighting in walk in cooler was 2 Foot candles. Please repair so that lighting is 10 foot candles at 30 inches from ground. By next standard inspection.

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Ventilation system not maintained. Window air conditioner in kitchen had visible residue on fan guards at time of inspection. Please clean by 04/20/22

Discussed the monitoring of the walk in cooler temperatures.
Discussed the gasket on the three door cooler that is used for storage in kitchen, when it is no longer smooth, durable and easily cleanable it will need repaired or replaced.
