Repeat Physical facilities not maintained in good repair. Wallpaper by drivethrough peeling back exposing wallboard. Please repair before next standard inspection.

Presence of live insects. Fruit flies observed in storage area by back door, around utensil storage, mop sink and under produce sink. Facility has pest control agreement.
