3717-1-04.4(N)(3) / Manual and mechanical warewashing equipment, chemical sanitization (quaternary ammonium) - temp., pH, concentration, and hardness
Critical Corrected During Inspection Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness. Observed 3 buckets of quaternary ammonia sanitizer less than 200ppm. PIC had all buckets remade at time of inspection.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. Observed spatula stored in the clean coffee corner to have visible residue. PIC had cleaned at time of inspection.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Repeat Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed 3 lbs of salad mix with internal temperature of 46F and 5 oz of whipped cream with an internal temperature of 44F in the room service make line. PIC voluntarily discarded at time of inspection.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Corrected During Inspection Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed foods in the dairy cooler to have no datemark. PIC verified milk was opened on Friday. Ensure TCS held for more than 24 hours have a datemark. PIC corrected at time of inspection.

3717-1-04.1(Y) / Temperature measuring devices.
Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable. Observed no thermometer in the right room service beverage cooler, and thermometer in the back of the cooler of the left cooler. Obtain new thermometer and relocate thermometer to warmest part of the left cooler by reinspection 10/3/2023.

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Repeat Corrected During Inspection Working food containers not properly labeled. Observed no common name in open food products in the catering and meat walk-in coolers. PIC had correct at time of inspection.

3717-1-06.4(K) / Controlling pests.
Critical Repeat Presence of live insects. Observed small flying insects in the following locations: behind unused sink in the room service cook area, under the rethermilizer/boilers, behind digester in the warewashing area, near the chemical rack in he warewashing area, and behind the conveyor warewashing machines. Contact licensed pest control and rid all evidence of flying insects by reinspection 10/3/2023.

3717-1-03.2(U) / Food display - preventing contamination by consumers.
Critical Corrected During Inspection Food display not properly protected from contamination by consumers. Observed customer touch english muffin with bare hands at the self service station. Tongs were provided. PIC intervened and discarded the rest of the English muffins at time of inspection.

3717-1-02.3(C) / Hair restraints - effectiveness.
Food employee(s) not wearing a hair restraint. Observed improper hair restraints in use at time of inspection. Hair restraints can take the form of hair bands, bandanas, hats, beard/hair nets accompanying complete restraint of hair.Including servers who plate biscuits/salads/make speciality drinks etc. hair restraints are required as for these activities because they meet the definition of food handling employees. Correct by reinspection 10/3/2023.

3717-1-04.8(E)(1) / Single-service and single-use articles - storage
[52] Improper storage of single-service and single-use articles. Observed lids for cups to be stored not inverted or covered in the room service area. PIC had lids covered at time of inspection.

3717-1-04.1(A) / Equipment and utensils - durability and strength.
Repeat Non-durable equipment observed. Observed room service cook make line (first cooler) to have an ambient of 46F. Ensure all cooling equipment used to store TCS foods function to keep foods 41F or below. Correct by next standard inspection.

3717-1-04.3(A)(1) / Equipment, clothes washers and dryers, and storage cabinets - contamination prevention location
Equipment or cabinets used to store cleaned items placed in an unapproved location. Observed condensation accumulation from the pipes of the condenser unit in the walk-in freezer. Ensure no foods are located where potential contamination may occur. Correct by next standard inspection.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Equipment components are not intact, tight or properly adjusted. Observed gasket on the desert cooler in the room service to be torn, rendering the surfaces no longer smooth, durable, non-absorbent, or easily cleanable. PIC stated the gasket is on order. Correct by next standard inspection.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Corrected During Inspection Equipment components are not intact, tight or properly adjusted. Observed metal shaving in both of the can openers. PIC stated they would discontinue use of the can openers until new blades can be obtained.

3717-1-04(I) / Nonfood-contact surfaces - materials.
Nonfood-contact surfaces constructed of unapproved materials. Observed tape and plastic sheeting being used to cover the old return tray opening the way in the warewashing area. This renders the surfaces no longer smooth, durable, non-absorbent, or easily cleanable. Correct by next standard inspection.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed shelving in the room service area holding dry stock and ice to have visible dust accumulation. Clean by reinspection 10/3/2023.

Corrected During Inspection Non-food contact surfaces of equipment are unclean. Observed shelving of the condiment station to have visible residue. PIC had cleaned at time of inspection.

Corrected During Inspection Non-food contact surfaces of equipment are unclean. Observed bottoms of the freezer in the A30 kitchen, bottom of the refrigerator in the A20 kitchen, and the cabinet under the coffee machine in A47 kitchen to have visible residue. PIC corrected at time of inspection

Corrected During Inspection Non-food contact surface(s) not cleaned at the required frequency. Observed exterior of the soda dispenser ice shoots in the cafe area to have visible residue. PIC had cleaned at time of inspection.

Non-food contact surfaces of equipment are unclean. Observed shelves of the walk-in catering cooler to have visible residue. Clean by reinspection 10/3/2023.

3717-1-05.1(J) / Backflow prevention device - when required.
Critical Corrected During Inspection No air gap or approved backflow prevention device on the plumbing system. Observed various new coffee machine throughout the facility to not have a backflow device installed. Richland Public Health Plumbing Department was informed and will be reaching out to schedule an inspection. PIC stated they will be discontinuing use of the coffee machine until back flow devices and inspection may occur.

3717-1-05.1(D) / Backflow prevention - air gap.
Critical Corrected During Inspection Insufficient air gap between the flood rim and the water supply inlet. Observed no air gap in the A20 kitchen under the ice machine. Per Ohio Uniform Food Code, "An air gap... shall be at least twice the diameter of the water supply inlet and may not be less than one inch". PIC had corrected at time of inspection.

Plumbing fixtures unclean. Observed the drain by the coffee corner to have visible residue accumulation. Clean by reinspection 10/3/2023.

Facility not maintained clean. Observed floor behind the digester in the warewashing area to have visible residue. Clean by reinspection 10/3/2023.

Facility not maintained clean. Observed the wall under the room service coffee machine to have visible dust accumulation. Clean by reinspection 10/3/2023.

3717-1-06(B) / Outdoor areas - surface characteristics
Improper outdoor surface characteristics. Observed acoustical ceiling tile in the pizza area, rendering the surface no longer smooth, durable, non-absorbent, and easily cleanable. Correct by next standard inspection.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed ceiling tiles in the following locations to have visible residue: back corner of the warewashing area, and behind the conveyor near the tray return of the warewashing area. Clean by reinspection 10/3/2023.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed peeling paint in the walk-in meat cooler, rendering the surfaces no longer smooth, durable, non-absorbent, or easily cleanable. Correct by next standard inspection.
