3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical (CORRECTED DURING INSPECTION) Equipment food-contact surfaces or utensils are unclean. -Observed dried food spillage on the counter-mounted can opener and blade.
PIC removed to warewashing area at time of inspection.
3717-1-03.4(I)(2) / Time as a public health control - four hour time limit
Critical Repeat (CORRECTED DURING INSPECTION) Improper use of time as a public health control (4 hours). -Observed two (2) containers of shredded lettuce marked for discard at approximately 11:30 AM, still being used on the prep line at time of inspection (approximately 1:30 PM).
PIC discarded remaining product into trash and provided verbal instruction to food employees.
3717-1-06.4(K) / Controlling pests.
Critical (CORRECTED DURING INSPECTION) Presence of live insects, rodents, and other pests. -Observed area below drive-through beverage filling stations to show evidence of pest activity.
PIC provided RPH sanitarian with proof of service from licensed pest control operator (PCO) detailing service to location on 9/19/2018.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. -Observed non-food contact surfaces in facility which were not cleaned at a frequency necessary to keep them in a cleanly state. Special attention to fryer sides, lower prep table shelves, oven/equipment doors, seals, corners and seams of equipment where grease and debris are prone to accumulate.

Plumbing system not properly maintained or repaired. -Observed the drain cover below the unused salad rail table to be missing. -Observed the drain lines below the drive-through beverage dispensing stations to be visibly dirty and covers to be askew.
