Corrected During Inspection Observed accumulation of soil residue on nonfood-contact surfaces of the cover over the garbal for the three compartment sink. Cover was covered in plastic which was torn and holding food and water.

3717-1-05.1(D) / Backflow prevention - air gap.
Critical Corrected During Inspection Observed no air gap between a plumbing fixture and a piece of equipment that was not sufficient to prevent back siphonage of contaminated water. Sink across from the stove was being used as a prep sink. It is not air gapped. Tim Day stated they will no longer use this sink as a prep sink, but will use it as a hand sink. The have another properly plumbed prep sink they can use.
To prevent contamination, an air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment, or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than one inch (25 millimeters).
3717-1-06.4(A) / Repairing.
The physical facilities are not being maintained in good repair. Broken tile in fron of the tilt skillet.

Observed a build-up of dirt and debris on the wall and floor around the tilt skillet.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
3717-1-06.4(K) / Controlling pests.
Critical Corrected During Inspection Observed the presence of live insects and a bug zapper over a prep table. Bug Zappers may be used only in an area where there is no chance of cross contamination
To prevent illness, the presence of insects, rodents, and other pests shall be controlled by: Routinely inspecting incoming shipments of food and supplies; routinely inspecting the premises for evidence of pests; Using methods, if pests are found, such as trapping devices or pesticides used according section (C), (K) and (L) of rule 3717-1-07.1 of this code; and eliminating harborage conditions.
