Facility not maintained clean. -Observed Plastic Air Flaps in Meat Dept. to have stickers and debris on flaps. -Observed splattered dry milk residue splattered on walls through out dairy cooler. -Observed dust on fan vents located in dairy cooler. -Observed ice build-up on fan guards located in dairy cooler. -Observed dust around ceiling ventilation located in Deli/Bakery.

Food process reviewed: Sushi, checked calibration of probe and reviewed the process for acidification of white rice.
Conducted inspection with PIC. Reviewed inspection findings with PIC. Retail Food Establishment does following Risk Level IV activities: Offers menu item raw and TCS meats, poultry products, fish and shellfish.
-Discussed: Brown rice shall be kept cold during preparation.
- Discussed Proper load levels in coffin coolers, to promote proper cooling of units(limit stacking products).
- Provided PIC a Meat label species chart.
