3717-1-05.4(H) / Toilet room receptacle - covered.
Repeat Observed no covered receptacle in women's restroom.
A toilet room used by females shall be provided with a covered receptacle for sanitary napkins. Correct By: 29-Dec-2017
3717-1-06.2(E) / Handwashing signage.
Corrected During Inspection Observed no handwashing sign(s) posted at handwashing sink(s) in the restroom used by employees. Provided PIC with RPH decal hand wash signage at time of inspection.
A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees.

PIC has signed copies of employee illness agreement on file.
-Continue to maintain these on the premises and have any new employees sign one as well.

PIC has RPH written response procedure for a vomit/diarrhea incident.
-Keep this posted or in an accessible location in the facility.
-Gather all supplies listed under step #2 of the above document and maintain in an accessible location at all times.

PIC states the water currently being used in the facility is hauled onto site from an approved potable source, per Ohio EPA temporary approval. A new PWS will be installed during the upcoming year pending Ohio EPA approval. Instructed PIC to notify RPH when this situation changes as a request will be made to verify most recent test results at time of future inspections. Sanitarian can be reached at: (419)632-0337.

Two (2) Slicers are presently used in the deli area. PIC states one is used for meats, and one is used for cheeses. Per PIC, both slicers are disassembled and treated to a wash, rinse, and sanitize procedure at the 3-compartment sink at a frequency equal to every four (4) hours or less.

PIC is certified as a Level Two (2) Food Protection Manager by ODH. Discussed Level One (1) Food Handler training for staff.
