Corrected During Inspection No written procedures for vomiting or diarrheal events. Provided ODH's guidance for vomit/fecal accidents and list of ingredients for a clean-up kit at time of inspection.

3717-1-03.1(A)(3) / Sources - packaged food labeling
Critical Corrected During Inspection Packaged food not properly labeled. Observed a canned food item with label missing. PIC moved to damaged goods area.

3717-1-03.6(A) / Discarding/reconditioning unsafe, adulterated, or not honestly presented food
Critical Corrected During Inspection Unsafe, adulterated or not honestly presented food not discarded or properly reconditioned. Observed several cans with dents on the top or bottom seam. PIC moved items to damaged goods area at time of inspection.

3717-1-07.1(K) / Rodent bait stations.
Improper use of a rodent bait station. Observed a set mouse trap under a food shelf. Pest control must be done through a licensed applicator. Observed use of covered rodent bait stations throughout the facility. Manager stated pest control is done through a licensed applicator. Discontinue use of set mouse traps in facility. Correct by: April 10, 2019.

3717-1-06.1(M) / Outer openings - protected.
Outer opening not protected. Observed back receiving garage door that had a gap on the bottom corner. Correct by: April 10, 2019.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Improper storage of food items. Observed slim jim containers, a package of crackers, a bottle of vegetable oil, and 2 liter pop bottles stored on the floor. Food items must be stored at least 6 inches off the floor. Correct by April 10, 2019.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed right door gasket on the pizza freezer coming loose and in need of repair. Correct by April 10, 2019.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Unclean nonfood-contact surfaces. Observed kick plates on the bottom of the freezers and cooler in need of greater cleaning frequency. Correct by April 10, 2019.

3717-1-06.4(F) / Drying mops.
Repeat Mops dried improperly. Observed mop in mop bucket in the back storage area. Mops should be hung to air dry. Correct by: April 10, 2019.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed bathroom floor in need of greater cleaning frequency. Correct by: April 10, 2019.

Comments
Conducted and discussed inspection with Marie Strahan, Manager, and Joette Moore, R.S. At the time of inspection, a mop sink with a clogged drain was observed in the basement that was not currently being used. Plumbing inspectors were notified of this issue. Standing water was observed in the basement at the bottom of the stairs. Manager states water comes through a hole at the bottom of the steps at the floor/wall juncture during heavy rains. Manager also stated that the roof is leaking in multiples places in the facility. Observed mold on the bottom of the walls in the bathrooms that are not currently being used. Environmental Health Director was notified of these issues. See pictures below of mop sink, bathrooms, and water in the basement.
