Critical Person in charge not ensuring proper cooling. PIC did not document cooling times and temperatures to verify proper cooling times and temperatures. Document cooling times and temperatures and retain for specialist review on 4/6/22.

3717-1-04.4(N)(3) / Manual and mechanical warewashing equipment, chemical sanitization (quaternary ammonium) - temp., pH, concentration, and hardness
Critical Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness. Two of the sani buckets were below 200ppm. PIC corrected during inspection.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Repeat Nonfood-contact surfaces not easily cleanable. Observed surfaces of the 3 hood vent systems to contain chipping paint creating a surface not smooth or easily cleanable. Two of the three hoods have been repainted. Have the third hood painted by 4/6/22.

Repeat Waste receptacles or handling units not cleaned and maintained. Observed grease build up on outdoor grease bin lid and side of dumpster. A cleaner has been added to the grease on the grease bin lid and side of dumpster. Person doing the clean-up is supposed to do another treatment and then clean up. Correct by 4/6/22

Repeat Improperly cleaned storage area for refuse, recyclables, or returnables. Observed outdoor dumpster area surface to contain grease spillage. Treatment has been added to concrete. The person doing the clean-up is supposed to do another treatment and then clean up. Correct by 4/6/22

Repeat Facility not maintained clean. Observed fan guards in walk-in cooler and walk-in freezer to contain dust. PIC cleaned the outside of the fan guards but there is still build-up on the inside. Remove dust build-up by 4/6/22

Repeat Physical facilities not maintained in good repair. Observed areas throughout facility with low floor tile grout creating surfaces that are not smooth or easily cleanable. The grouting was started and is contracted to be completed. Correct by 4/6/22

Written procedures and plans are not maintained and implemented. Facility has written procedures for sliced American cheese variance, however procedures were not being followed or monitored properly. PIC stated facility is using time-in-lieu of temperature for multiple TCS products, however no procedures were on file and products were not time marked. Provided time-in-lieu of temperature procedure for PIC to complete and keep on file. -Correct by 4/1/22

PIC states facility is using time instead of temperature control for multiple food products, such as sliced cheeses, tomatoes and cheese sauce, with no written procedure on file. Provided procedure template for facility to complete and keep on file for products using time instead of temperature control. -Correct by 4/1/22

Presence of live insects. Observed insects crawling on food prep sink floor drain. PIC instructed employee to clean area. PIC provided most recent pest control service report from 3/16/22. Discussed working with pest control company for additional service and maintaining area in clean condition. -Correct by 4/1/22
