Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable. Observed: Person in Charge could not locate thermometer located in Walk in Cooler. .
Unit shall have a workable, Easy to read thermometer. located in the warmest portion of cooling unit.
3717-1-03.2(D) / Food storage containers - identified with common name of food.
Working food containers not properly labeled. Observed Ketchup bottles on table that wasn't properly labeled with common name.

3717-1-06.1(M) / Outer openings - protected.
Outer opening not protected. Observed screen door open in back entrance of building.

Repeat Floors, walls, and/or ceilings not smooth and easily cleanable. Observed missing ceiling tile's through out kitchen area.

Physical facilities not maintained in good repair. Observed Duck tape on back of ice machine to hold filter on place.
Action: Duct tape is prohibited in kitchen.

Facility not maintained clean. Observed dust around ceiling tile and metal conduits.
