The plumbing system was not properly repaired. OBSERVED toliet not working properly in Ladies restroom. Employees shall have a restroom in good repair to prevent a foodborne illness from having to mess with toliet after use..
To prevent health hazards, a plumbing system shall be repaired according to the Ohio building code. Correct By: 29-Apr-2015 The restroom is repaired & working properly.
3717-1-06.4(B)/Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris through out the store on the floors & storage area in back. Floors shall be swept & cleaned to prevent a build up of dirt/ debris which could contaminate food & cause a food borne illness. Clean floors through out facility.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Correct By: 29-Apr-2015 Floors were clean & polished at the time of reinspection

Observed mops being dried using an unapproved method. Mops shall be hung up to dry to prevent the growth of pathogens which could cause a foodborne illness. Supply some way of hanging mops in Mop sink area
After use, mops shall be placed in a position that allows them to air-dry without soiling walls,equipment, or supplies. Correct By: 29-Apr-2015 Mops were hanging as required
