3717-1-02.4(C)(13) / PIC: duties - ensure employees are not contacting foods with bare hands
Critical Person in charge did not ensure employees are preventing cross contamination of foods with bare hands by using suitable utensil. PIC stated toppings in the top of the make table coolers are used on salads and pizzas. Gloves are used when making salads but are not used when making pizzas. Discussed using separate toppings for salads to prevent bare hand contact with ready to eat food items. Correct by: 4/28/22.

3717-1-02.4(A)(2) / PIC - Manager Certification
Facility does not have an employee with manager certification in food protection. Manager's certificate expired on 2/13/22. Correct by next standard inspection.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. Salami, ham, pepperoni, and chicken had an internal temperature of 51*F in the top of the pizza make table cooler. PIC voluntarily discarded items above 41*F at the time of inspection. See voluntary destruction form for product amounts.

Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. Container of salad in the bottom of the pizza make table cooler had an internal temperature of 47*F. PIC voluntarily discarded salad at the time of inspection. See voluntary destruction form for product amounts.

Critical Corrected During Inspection Ready-to-eat, TCS food not properly discarded when required. Observed a container of sliced american cheese in the bottom of the pizza make table cooler with a prep date of 3/31. PIC voluntarily discarded approximately two pounds of cheese at the time of inspection.

Improper storage of food items. Observed two containers of pizza sauce setting on the floor inside of the walk-in cooler. Correct by: 4/28/22.

3717-1-03.2(K) / In-use utensils - between-use storage.
In-use utensils improperly stored. Knives used for cutting subs were being stored in between the make table coolers. Correct by: 4/28/22.

Repeat Equipment components are not intact, tight or properly adjusted. Ripped gasket on the top door of the dough proofer. Correct by next inspection.

Critical Corrected During Inspection Food contact surfaces not easily cleanable. Observed a cracked plastic container in the pizza make table cooler. PIC voluntarily discarded cracked container at the time of inspection.

Non-food contact surfaces of equipment are unclean. Observed a build-up of residue on the dough carts inside of the walk-in cooler. Correct by: 4/28/22.

Corrected During Inspection Plumbing system not properly maintained or repaired. Three compartment sink faucet leaking around the hot water handle. PIC adjusted at the time of inspection.

Repeat Physical facilities not maintained in good repair. Sections of coving missing by the office and the dish washing area. Correct by next inspection.

Comments
Conducted inspection with Jen Lacey, EHSIT. Discussed inspection with manager Kirstan. Ambient air temperature inside of the walk-in cooler and pizza make table cooler was 43*F at the time of inspection. Discussed adjusting coolers to cold hold at 41*F or below. Re-inspection to occur on Thursday, April 28, 2022. Emailed inspection report and voluntary destruction form.
